Jinarc® Risk Management Program: Data Privacy Notice


Introduction

This Data Privacy Consent Form is designed to ensure compliance with applicable data privacy laws and regulations in the Philippines, including the Data Privacy Act of 2012. By signing this form, you, as the prescribing physician, provide your consent for the collection, processing, and sharing of personal and medical data related to you and your enrolled patients. This is necessary for safety monitoring and other related data processing activities as part of the Jinarc® Risk Management Program (RMP).

Purpose of Data Collection

The data collected will be used solely for the administration and monitoring of the Jinarc Risk Management Program, including but not limited to:

  1. Enrolling and managing patients prescribed Jinarc.
  2. Monitoring patient safety, including proper timing of liver function tests and adverse event reporting.
  3. Ensuring proper delivery of medications to the patient’s preferred address.
  4. Communicating with you regarding any reported adverse events or program updates.

Data to Be Collected

For Prescribing Physicians:

  1. Full Name
  2. Hospital Affiliation
  3. Email Address
  4. Contact Number

For Patients:

  1. Full Name
  2. Age
  3. Sex
  4. Date of Birth
  5. Mobile Number
  6. Address for Medication Delivery
  7. Prescription
  8. Liver Function Test Results

Data Sharing and Retention

  1. Processed and stored securely by Otsuka and its authorized partners.
  2. Shared with regulatory authorities, if required, for compliance with pharmacovigilance and other legal obligations.
  3. Retained only for as long as necessary to fulfill the purposes of the program or as required by law.

Rights of the Data Subject

As a data subject, you and your patients have the following rights:

  1. The right to access, correct, or update personal data.
  2. The right to withdraw consent at any time, subject to legal or contractual obligations.
  3. The right to lodge a complaint with the National Privacy Commission if you believe your data privacy rights have been violated.
